Here’s a weird one….

42M R shoulder pain anteriorly for several months, no antecedent trauma or prior surgery, suspect biceps or subscap pathology.

There is a large well defined gas collection surrounded by what appears to be fluid within the subcoracoid joint recess. No other joint fluid, no labral tear.

Is this gas in an unusual synovial cyst? A T2-bright lesion that contains gas, either within or outside the joint?

Spoke to surgeon, and while an MR arthrogram might be entertaining and help answer these questions, ultimately he needs to be scoped. I will follow up once he sends me post-op pictures.

Any additional ideas welcome.
