trochlear OCD?

I had a similar case of a trochlear OCD recently in a 14-year-old (see attached) which looks pretty similar. I think that these heal with rest but would be a little hesitant at him going back to full activities if there is still BME and enhancement, although I guess the lack of symptoms is reassuring.

Hi everyone,
I was wondering if I could get input on this case – has anyone ever seen a trochlear OCD?

14 year old boy active in tae kwon do and tennis as well as playing bassoon. He was experiencing pain with extension of his elbow laterally and clicking in March of this year.

He finally got an MRI in June where it was reported as a possible trochlear OCD and follow up recommended. He stopped the tennis and tae kwon do and the pain got better.

Re imaged 4 months later in October and there is mild increase in marrow edema. Do people think it is just an OCD? Would you recommend resumption of the activities, tennis and tae kwon do?

The second imaging done in October has post contrast. The non fat sat images are T1 fat sat are STIR and the last slide is post con (labelled).

Thanks for your thoughts!

Best,
Uma
