several replies to the case below, mostly invoking some sort of chondroid lesion.
Best reply was from Mark Murphey:
"Believe this is an intracortical chondroma (exchondroma) as I spoke about at SSR this year. Tongue superiorly on lateral radiograph great for jntracortical location as is signal and largely peripheral enhancement." (italics and bold mine)
Point being: If I had been paying more attention or not chatting during his SSR presentation I would have known this!

Attached powerpoint includes ortho clinic note, XR, and MR of knee with Gad showing a cortically-based distal femoral lesion (anterior) which corresponds to patient’s pain after an acute fall.
Looks like a non-aggressive lesion like osteochondroma, but what is that vertical line extending superiorly through cortex on sagittals? No marrow edema or swelling so hard to believe it’s an acute fx…..
