    My 2 cents,
    Looks like chondroid matrix in lesion and in a patient with pain and a “hot” bone scan, I would be concerned that this may be a low grade chondrosarcoma. Because of symptoms and x-ray plus h/o breast cancer, I think a biopsy would be prudent as metastatic disease would also have to be considered but I favor a chondroid lesion as above.

    It seems tom me that the history indicates that older images are available in radiology or radiotherapy PACS.

    Considering the medical history and the mixed pattern, I would go for a biopsy and consider a breast carcinoma metastasis as first diagnosis unless proven otherwise.
