I think this is OA due to subchondral small biarticular geodes of left CMC area of pain
Both ulnar prestyloid recesses and lt styloid tip show no erosions to suggest RA. MCP joints also negative. Possible tiny erosion of rt ulnar styloid.

I think these are just extensive degenerative cysts. Sometimes they can extend a long way into the bone and appear quite bizarre.

It’s a degenerative cyst (geode), they can dissect surprisingly far from the subchondral bone
4.
Arthritis and marrow edema which has an unusual distribution due to underlying osteoporosis. Could also be an intraosseous ganglion.
5.
Large subchondral cysts (geodes) related to the 1st CMC OA.
