Sorry it took a while to get back to the list.There were several replies regarding this case. The consensus was delaminating injury of the medial prepatellar quadriceps continuation.

Knowing the correct keyword to search certainly helps!

There is a fluid crescent interposed between the crural fascia and patella medially which I am thinking relates to stripping of the fascia from the patella associated with VMO strain, but I don’t believe I’ve seen this pattern before. With exception of a very small partial tear involving far superior fibers of the MPFL (seen on only one slice) the patellar attachment of the MPFL appears intact.
