Strange case I’d love to get an opinion on

Hi all,

I apologize in advance for the image quality, this comes from a small rural hospital with a low field strength magnet where we have no protocol control!

That caveat aside, I’d love some thoughts on this case.

When I first opened it and looked at the T1 I thought I was looking at something aggressive like osteosarcoma.

But once I started looking through it I *think* it’s a prox tib fib capsule ganglion run wild…my impression is that it’s a ganglion with extensive osseuous retention at the fibular head and then it found it’s way into the extensor digitorum longus tendon at its tibial origin and decompressed all the way to the ankle. Distally it seems to break free of the tendon and cause some peroneal denervation change. I’ve seen peroneal perineural ganglia from the tib fib capsule but never this…so before I sign it as such I’d love to hear if anyone has other ideas!

The only comparison I have is a radio graphic study from 2017.
