So, the radiographs fell into the reading list, and I called the ordering. She fell in her yard, twisting her knee and they were concerned for ACL tear. She had no prior imaging.
Against advice, they increased the FOV of the knee MRI. For surgical planning, a tibfib MRI was ordered a few days later.
My dx: Osteofibrous Dysplasia vs Adamantanoma.
Area in the POSTERIOR tib looks NOF’ish.
However, the entire picture, particularly the cortically based areas that extend superficially concern me which is why I called.
Could this POSSIBLY be an intraosseous vascular lesion? Would you add that ddx? I remember biopsying a lesion of the tibia similar to this (but much more focal) under MR. Had to call the surgeon as the thing bled like stink.
