There is no acute fracture. This is an obvious, chronic, unstable
non-union scaphoid waist fracture with sclerosis but no collapse of the
proximal pole. We can consider possible AVN, though I never know how to
differentiate AVN without collapse from reactive sclerosis in a chronic
non-union.
My main question is about the staging of SNAC….or, by extension, SLAC
wrist.
As I understand it, the first stage is Radioscaphoid DJD.
In this instance, the radioscaphoid cartilage space appears relatively
preserved.
Yet there is lateral and proximal migration of the capitate over the
widened SL interval with midcarpal joint space narrowing, which should be
end-stage (stage 3).
Maybe the staging system is useless?
Is it even Clinically meaningful?

That raises another issue.
I have received several inquiries about the OCAD Multidisciplinary Lecture
Series, which went suddenly quiet this last week.
We actually have a committee that has worked to coordinate these lectures,
and they overwhelmingly agreed that most people are experiencing Zoom
fatigue.
Moving forward, we will be having one lecture a month, in addition to
collaborating with the Rio MSK Imaging Group on monthly Case Presentation
Sessions (all of which are archived in the Video section of our website,
ocadmsk.com).
We have lectures scheduled through September, but are currently working to
invite clinical lecturers for October-December and beyond.
What distinguishes this lecture series is that we are focusing on topics
presented by our clinical colleagues, which are of interest and relevance
to MSK radiologists.
That means that most of our lecturers have been orthopedists and
rheumatologists, with some pathologists and others…..but we are
definitely open to creative inclusion.
If you have a clinical colleague or friend or former associate….or
spouse, significant other, drinking buddy, you name it—-who you think
would be able and willing to give us a great lecture, please contact me to
discuss.
All lectures scheduled from October onward will be on the 2nd Thursday of
each month at Noon Eastern Time.
IF they are unavailable for time slot, we would consider pre-recorded
lectures, provided they could be available for 10-15 minutes Live Q&A.
