Congratulations to Dr. Harun Gupta who mentioned Hemophilia as a 2nd possibility.
I have seen many-many hemophilia cases but all with cartilage destruction
This one had some nice features-
– earlier presentation with synovial hypertrophy and hemosiderin staining all over
– incr signal on opposed phase- typical of that staining in joint linings/synovium and biceps sheath
