OCAD Tendon changes in a diabetic after toe amputations (MRI / foot)

Hi there everyone,

this is a 70 year old male diabetic. Imaged to r/o osteomyelitis and abscess at the forefoot. He has had his 5th toe amputated 9 yrs ago, the distal phalanx of the great toe 8 yrs ago, and his 2nd toe 1 year ago.

I’m puzzled by the extensive changes at the FHL tendon, which were already there on some prior scans 7 yrs ago. There seems to be a tear of the distal FHL tendon just proximally of the MTP joint.

In addition the current scans show shorter segment distal changes at the 2nd, 3rd and 4th tendon. Are these all related to the amputations? I would have expected atrophy though.
