I need your help with this case please.
M69 chronic pain at the left hip.
There’s marked labral degeneration bilaterally and especially on the left.
Prominent osteophytes at the acetabulum with something ressembling a
pseudoarthrosis at a particularly large lateral one, as well as some loose
bodies.
I don’t see much “classic” OA as such, the cartilage looks pretty good for
his age, as do the femoral heads (except for the “bumps”).
Are the labral and bony changes secondary to chronic FAI? There seems to be
a cam-type deformity of the proximal femur on both sides.
Or maybe traction related? The rectus insertion looks fine though. Some
form of hyperostosis?
What is your impression?
