(OCAD) Atlanto-axial instability, rheumatoid changes? (cervical MRI, M83)

Hi all,

this is an 83 year old male patient with nuchal pain and restricted ROM for
several months already. Denies trauma.

There are changes at the atlanto-axial joints suggesting chronic
instability (unfortunately poorly imaged due to focus on the usual
degenerative changes).
Is that “pannus” at the odontoid process? Could all these changes be
secondary to rheumatoid arthritis?
Would you consider infectious arthropathy, notably TB, as a possible
differential?
Would further imaging (CT) be useful?
