Hello,

Happy 4th all!! Enjoy a photo with my 85 yr old parents celebrating the independence day!

Congratulations to
Drs. Roy Gottleib, Damon Sacco, Pritish Bawa, Kulvinder Singh, Cyrus Golcaz, and Xianjing Peng.

This is a complex tear with several components- calcified flap in med gutter (see xr/mr attached) and complete radial near the root and horizontal flap components

Mimics of bucket are-

-chondrocalcinosis (as in this case xray attached- looks like fragment in notch on MRI)

  • – meniscofem lig
  • – air

also attached is how bucket looks like on 3D and arthroscopy for those not used to axial 3D MRI!

Responses are pasted below!

Looks like a complex tear with predominant undersurface horizontal tear, large posterior horn radial tear and a flap in the meniscofemoral recess.

Looks complex with flap, I don’t see the bucket on the 3d axial provided.

Complex

I would say it’s complex (option B).
Flap tear of posterior horn / root, hoop stress, extrusion of torn body and torn fragments of posterior horn / root lying adjacent to posterior junction and posterior part of body of MM.

c. bucket handle

b. complex tear

Best!
