I read this case two weeks ago as a sprain and partial tear of the proximal accessory ulnar collateral ligament with associated MC head contusion, normal UCL. Two weeks later the orthopedic surgeon called me that it made sense because he was loose to extension across the ulnar aspect of the joint, but that he strangely was stuck in mild extension across the joint and that even with passive manipulation on exam he couldn’t flex across the MCP.
Over FaceTime I watched him examine the patient and then on further review of MR images with this context I noticed that the ulnar sesamoid appeared to be in a locked configuration. I’m reading case reports of this, so I’m sure some of you have seen it, but since it’s new to me I thought I’d share!
