Thanks for all your answers:
3 people are in favor of chordoma: two suggest MRI with contrast and DWI, and one suggested biopsy (ot-rads 4).
1 person suggested post-traumatic / stress reaction and instability (althought I don’t have any notions about pain nor traumatic event) versus benign notochordal rest (if no enhancement).
I’ll probably refer the case to a hospital for overall care and biopsy.

Dear all,
I’d like to have your opinion on this lesion I found in the coccyx of a patient who came for a ankylosis spondylitis sacro-iliac MRI. I wanted to say coccydynia but it’s kinda broad and I don’t want to miss a chordoma or anything else. Any suggestion ?
