Incidental Femur Lesion on XR in 38M 20+ years pain, XR and MRI, Please Help

The XR was reported (not by me) as suggested of Pagets.
I don’t think this is Pagets and the MRI does not support that….would be
exceptional in a 38 year old….especially these days as Pagets is less
common than it used to be (do we know why that is, by the way?).
To me these look like subcortical vascular channels…but it doesn’t look
typical of cases of hemangiomatosis I’ve seen that have affected bone and
soft tissue….but is it? If not, what is it? I’m sure someone knows.
I’m waiting to finalize the report…would love if you could advise me soon.

    One more thing….I suspect the chronic patellar maltracking might have
    more to do with his knee pain for more than half of his life. I think this
    bone lesion is incidental.
