Definitely not Charcot in my opinion.
This falls into the category of severe remodelling (without the gross
destruction or dislocation you expect with Charcot) with convex glenoid and
concave humeral head resulting in the reverse of the normal configuration.
I am fairly certain that it must be either developmental dysplasia or an
early childhood insult such as trauma and/or humeral AVN – and this would
be the shoulder equivalent of hip acetabular dysplasia, Perthe’s or slipped
epiphysis –
and then secondary adult OA.
