Differential diagnosis help.

I hope everyone is well!

I’m reading a case from a site we help decompress MSK MR and have a strange one to share.

46 yo with “mass on hand and wrist growing for over 10 years, lots of pain radiating through entire right upper extremity.”

Given a lack of any enhancement I’m thinking this is bad lymphedema but I have no history of prior ax LN dissection. (Though I suppose it’s possible), NF, or other lymphatic obstructing entities (though that doesn’t mean there isn’t one). The 10 year history of “growing mass” also bothers me.

Any other things I should consider? Without enhancement something like dermatofibrosarcoma protuberans doesn’t make sense. Doesn’t look mass like enough for well diff’d liposarcoma.
