It’s a small series….but no benign notochordal rest enhanced, and all the chordomas did.
I recommended a contrast exam.
I’m hoping it doesn’t enhance.
Then I guess I’d have to decide whether to follow it up in a year or two….
I’ll share the contrast images when she returns.

She has mild scoliosis and some abnormalities on Lsp MRI that might explain LBP.The referrer was looking specifically for sacroiliitis, the SI joints are normal.
I suspect this dark T1 / bright STIR signal lesion in the coccyx is incidental.
I think the main DDx is benign notochordal rest vs chordoma….do you agree?
My understanding is that the notochordal rest won’t enhance but chordoma should….is that reliable?
(sometimes it’s two cases a day, apologies)
