I was asked to biopsy this lesion in the left 1st metatarsal in a 62 year old with recurrent metastatic breast cancer. No symptoms directly attributle to the lesion. 57 yr old female collegue experienced sudden onset nontraumatic pain on a long steep uphill walk. I reported MR as probable subchondral insufficiency # Follow up at 6 weeks shows abnormality is now bigger (though she is generally asymptomatic ) but still no bone ot cartilage collapse. TKR 3 years ago, chronic pain and stiffness in 69F. “Rule out partial Quadriceps tear” MRI. Please Help

[image: 1.jpg] Referral asked “r/o partial quad tendon tear”; maybe it’s scarred, but is not torn. There is a low lying patella. I see this dark signal thickening posterior to the patella which contacts the deep surface of the patella and seems to extend to the anterior tibial margin…though it seems separate from the patella
