Hi there, this is a 42 year old man with medial knee pain for several weeks. Explicitly denies trauma or any kind of trigger event. There’s this small osteochondral lesion with comparatively extensive bone marrow edema at the right femoral condyle. No meniscus pathology and no chondral defect. What’s going on here? Somewhat unusual case (at least I haven’t seen many if any before). I measured about a 4 cm gap. No other history given although I know that there can be a paucity of symptoms with this abnormality.
