Hi, I think you’re right about chondrosarcoma. There was a big british series of calcaneal lesions 3 yr ago, in which CS was rare, they had a few chondroblastomas but i read that cb kept to epiphyseal-equivalent calcaneal locations with lots of surrounding edema. https://pmc.ncbi.nlm.nih.gov/articles/PMC8553183/

I’d definitely be considering a chondroma given the imaging findings. There is narrow zone of transition posteriorly and ill defined anterior. Although the lesion is expansile, the cortex is not eroded. Whether that’s attributing to the soft tissue inflammation is a possibility. No definite soft tissue mass component that can be appreciated either. M N.
It is a cartilage tumor. Unusual site and there is likely infiltration – so chondrosarcoma.

How did this patient present? I’ll play devils advocate and say favored to be benign IF this is incidental. I’ve seen plenty of variations of intraosseous ganglia with enhancing granulation along the margins possible early transformation into a lipoma. Certainly on the far edge of the spectrum. If no pain I’d VERY closely follow it but wouldn’t argue with bx either. Cina Joseph Ali
