Thank to everyone who responsed. The mainstay of opinions was old/choronic infarction, other opinions included benign calcified endochondroma, desmoplatic fibroma, chondrosarcoma, chondromixoid fibroma,liposclerosing myxofibrous tumor. The powerpoint attached in original email provided more details including X-ray and more MRI images. The pathologic dignosis was consistent with osteoma. The surgeon said the lesion was very hard.

history of acute onset back pain Pretty similar to disc related epidural hematoma shared with us by Dr Phillip Tirman couple of months back, although not too much of disc disease in this case. Interesting 1. Acute denervation edema along L3 distribution (which can be expected).2. Contrast enhancement is little more diffuse than just peripheral. ?

Hi ocaders. Please help. MRI on a 58-year-old gentleman who came with the bone infarcts on x-ray. No underlying malignancy. Rest of the areas look like bone infarct but one does enhance in the distal femur metaphysis. The question I have is do the bone in facets enhance? Thanks for help in advance.
