Dear Ocaders,

I appreciate the feedback a lot.

Kay-Geert, Clarissa, Yoav, Atul (both cases 🙂👍), Avneesh, Gustavo,
Eugene, thanks a lot for opinions and references, you´re the best!

Concerning the acromial downslope case:
– consensus on not much literature support, but seems that in extreme cases
worth description.

About the hand bone marrow change case:
– most said non-bacterial osteomyelitis/ osteitis
– one SAPHO spectrum
– one bone marrow infiltration
– Clarissa came with JIA psoriatic type. I like particularly this
hypothesis ( doi.org/10.1148/rg.335125178 open source)

I asked about cutaneous and immune signs and symptoms by myself, and found
no significant information.
Patient was small (barely 1.5 M)

I´m leaning towards JIA or non-bacterial osteomyelitis/ osteitis and plan
to include both in the differential.

best regards,
Jair
