The case is impressive!! Why wouldn’t Hidradenitis suppurativa (aka Acne inversa) be a differential? This disease, while predominantly affecting the skin and subcutaneous tissue, has a predilection site at the intergluteal cleft (rima ani). I guess there was histology taken in your case, and it proved to be gout?
