Avulsion linea aspira?

My first post on OCAD! Looking forward to thoughts from the group. My practice is mostly oncology but this sports cases came in today from the hospital emergency room.

43 F with hx of sciatica and deg disc disease. She felt a pop in the back of her left leg 3 days ago descending stairs while holding a heavy object. Seen at another facility 3 days ago where the CT and X-rays were done. Sent home with pain meds. She re-presents with intractable pain and inability to walk. Now pain radiating to left flank and groin.

Thank you!

  1. OCAD

I think this is calcific tendinosis (hydroxyapatite deposition) at the gluteus maximus insertion. I have seen this degree of edema and chronic mild cortical erosion from calcific tendinosis at this site. I think the reported popping sensation might be unrelated to this imaging finding.
