41M work injury not specified 2010 and surgery 2011 MRI please help

Left Shoulder

Pt complains of bilateral shoulder pain with weakness, tingling and limited range of motion.

On the Left there is very mild insertional tendinosis without tear of the supraspinatus.
I suspect there was remote, limited subacromial decompression…there are no surgical anchor tracts.

On the Right there is AC DJD with periarticular edema, Supra and Infra insertional tendinosis and chronic superior labral tear with small paralabral cyst.

There is bilateral intramuscular fat surrounding the intramuscular tendon of the supraspinatus with apparent retraction of the central muscle at the MT junction.

Is this the long term sequela of remote MT junction tear?

I presume that must be what it is, but I have never seen this "degloving" pattern of chronic tear with intramuscular peritendinous fat signal.
