I included gout as a less likely ddx and suggested that lipid profile and serum uric acid would be helpful.
My attempt to reach the referrer was time consuming and useless, as expected.
Looks very much like this case including peritendinous intermediate signal soft tissue thickening

The history is Achilles tendonitis with mass
The patient is a 35 yo black maleIf it weren’t for the posterior peritendinous intermediate signal component, I would report xanthoma….is that peritendinous component typical of xanthoma?
I have not seen many cases.Thanks.
