I mentioned to Daniel that I had a similar case a year or so ago in a patient with excruciating pai. I did an ultrasound-guided barbotage, got calcium out and the patient’s pain was instantly better. This case of a 67-year-old male came through a few days ago, and looks very similar– the MRI was done 16 days after the xray.

I think this is hydroxyapatite crystal deposition disease (HADD) in the LCL. I have never seen one before.
