opinions were divided over this one.
Some opined that the prominent cysts and bone marrow edema point towards CPPD, while others said that the findings look unspecific.
Neuropathic arthropathy also comes to mind as it can manifest quite frequently at the Lisfranc joints, however I think it would lead to fragmentation and joint deformity, which aren´t prominent in this case.

Hi there everyone,
this is a 77 year old lady with chronic foot pain. The referring physician says she has CPPD. Doesn’t know about trauma.
MRI shows marked osteoarthritis at the TMT joints, especially II-IV, with marrow edema and large cysts. Also possible insufficiency / stress reaction at the proximal 2nd and 3rd metatarsals.
Alas, once again no xrays.
To me it looks like "ordinary" OA (some would call it "arthrosis") however prominent, possibly post-traumatic.
Or do you think it’s something more specific? Inflammatory?..Thanks, and have a good start this week!
