The majority of respondents favored an intramuscular cyst, complex cyst, or hematoma rather than a neoplasm.
We’ll keep you updated if we see follow up with a contrast study or pathology results.

59 year old man. 9 months of shoulder pain after injury at work. No relevant prior surgery.
The supraspinatus myotendinous junction contains a well-circumscribed lesion demonstrating T1 isointense signal to muscle, intermediate signal on T2, and a rim of low signal; we’re thinking possibly a prior interstitial tear with old hematoma/fibrous tissue, invagination of bursal fluid collection, or soft tissue neoplasm such as a myxoma.

We recommended repeating the exam with IV contrast. Any other thoughts would be appreciated!
