follow up MRI of 11F softball pitcher with proximal humeral lesion

I first posted this on Dec 20, 2023….in consult for my colleague Jonathan Sterman.Follow up MRI with and without contrast was recommended, but it was ordered non-contrast, I don’t think that matters.
I think there is definitely diminished marrow edema, but there is still this cortically based bright PDFS lesion….do you think it’s an incidental fibrous cortical defect, or something else?
Previous comments overwhelmingly favored avulsive stress injury, with one experienced pediatric radiologist urging caution since it was indeterminate and might be aggressive (osteosarcoma).
Can we agree this is simply stress, with interval improvement at 7 weeks?
Is there a reason to follow to resolution?
I don’t know if it’s necessary…and I am concerned that she will resume her sports activity and it will only look angrier again.
