40M knee pain after injury 5 months ago MRI Please Explain

There is a chronic meniscal ossicle embedded in the stump of the posterior horn medial meniscal root (red arrow) which is chronically avulsed (black arrow)
He got the MRI because he’s symptomatic, obviously, but there is no medial compartment cartilage thinning and no meniscal body extrusion—which is difficult to explain…..can you? What’s providing stability here?

Corresponding PDFS images…useless for seeing the meniscal ossicle, but you see the chronic root tear and the lack of extrusion and the absence of any stress related marrow edema (because it’s chronic)

Curious to hear your thoughts.
