Many comments suggesting Rapidly Progressive Osteoarthritis of the Hip
I remember all the presentations and reports a while back, but have never seen a case in practice.
This just looks so neuropathic to me, but I suppose it falls into that wastebasket diagnosis.
I do not have any history of recent joint injection….but that doesn’t necessarily mean anything.
Don Resnick mentioned that, among their cases, some had chondrocalcinosis, some didn’t…but they all did well with THR.

There is bone loss and articular impaction of the femoral head, possibly resulting from secondary osteonecrosis (scroll down to see prior XRs).
She has multiple fractures of the femoral head and anterior acetabular column, with bone fragmentation and debris and capsular distention, which looks like neuropathic arthropathy to me. There is faint callus at the acetabular fracture.
I’m curious what you think about the capsular and labral chondrocalcinosis….it’s very extensive (not sure it is as striking as on my viewer). Is this secondary? Or could this be pseudo-Charcot with CPPD? Can we tell?
14 months ago the left hip joint was narrow and there was mild flattening of the superolateral head, but no chondrocalcinosis
4.5 years ago she had a fracture of the Rt anterior acetabular column, but the hips looked normal
The referrer ordered the exam to evaluate fracture in a patient with osteoporosis
Her lumbar BMD confirms osteoporosis, but her hip measurements are in the normal range, currently
