It is actually prefemoral fat pad- not uncommonly involved in patellofem impingement / friction patients- with about 60% frequency.
It was 1st reported in my 2010 review article.
See attached images how it can get friction edema like in your case- adventitial bursa, pvns and bodies. Mostly, lateral aspect of the prefem fat pad is involved in impingements.
