Definitely not Charcot in my opinion.
This falls into the category of severe remodelling (without the gross destruction or dislocation you expect with Charcot) with convex glenoid and concave humeral head resulting in the reverse of the normal configuration.
I am fairly certain that it must be either developmental dysplasia or an early childhood insult such as trauma and/or humeral AVN – and this would be the shoulder equivalent of hip acetabular dysplasia, Perthe’s or slipped epiphysis –
and then secondary adult OA.

Charcot arthropathy hmm where’s the destructive features debris density etc. Seems there is preservation of the capsular tissues and osteo articular surfaces. Developmental with underlying neuroma defect maybe or metaphysical dysplasia but not the atrophic destruction I know as Charcot arthropathy. Maybe the spectrum is wider than I have learned and taught.!!?
