Interesting shoulder case with responses.

Thanks for everyone that responded. I reported this as probable
intraosseous HADD which I have never seen in this spot but as Mark Awh
said, It can be anywhere. So in this case possible AC joint calcific
periarthritis with intraosseous HADD.

My nephew Philippe Tirman, a radiologist, and I tried to write up
intraosseous exytension of rotator cuff calcific tendinitis years ago
(soundly rejected). What we found was a high preponderance of cystic
looking lesions with a rim of dark calcium like in this photo from this

I have spoken with the orthopedist who will report back his impressions
after reviewing the case in the next few days. I will report back.

Responses were as follows:

Subchondral infarcts x 3
Osteoid osteoma x 6
Osteonecrosis x 1
AVN x 1
Good ole Geode x 1
Intraosseous HADD x 7

Mauricio Dela Lama made a good point when he said: “The age made me go
against osteoid osteoma, but if it has been there for 20 years maybe could
still be OO.”
