What’s wrong with this knee?

Hi there,
this is a 42 year old man with medial knee pain for several weeks. Explicitly denies trauma or any kind of trigger event.
There’s this small osteochondral lesion with comparatively extensive bone marrow edema at the right femoral condyle.
No meniscus pathology and no chondral defect.

What’s going on here?

    Many thanks to all responders.
    Consensus was that this is most likely a subchondral stress / insufficiency fracture in the setting of chronic mechanical overload or systemic disease.
