Calling arthritis mavens….what is this???

Not quite so many “mavens” as I expected….and, apparently Dr. Stevens now knows what a “maven” is—-I thought it was part of common parlance. I used the word in the sense of “expert” rather than “connoisseur” (she found both synonyms).

Responses as follows:
1. MCTD covers all eventualities, but helps no one.
2. Calcification related to intramuscular injection perhaps?????
3. Prior electrocution injury (it’s a stretch)
4. Maybe lupus? or other collagenosis???
5. Don’t know, maybe dermatomyositis
6. Looks like dermatomyositis, entesitis and erosions in the shoulder, hoops and hands. Dermatomyositis can be associated with RA, Sjogrens, SLE, etc. I’m not one for catch all acronyms, but if you really want a unifying term, polymyositis associated with Arthritis would be the proper rheumatological catch all. I dislike the term MCTD. Does she have raised anti mp?

****I have no labs. I get the sense that the patient has not even been told she has any sort of arthropathy or connective tissue disorder. I will try to get follow-up in some weeks.

Finally, Dr. Atul Taneja shared images and references about Deltoid Contracture. I had never heard of this.
In my opinion, it looks exactly like what is going on in her deltoid….but in this patient, it is only a small component of the findings. It doesn’t explain the erosive arthropathy which is quite pronounced in her other shoulder and her right small MCP joint. I’m not sure if I included the images that show calcific deposits in her T Minor also…..and there was another small one in a paraspinal muscle near the TL junction as well as bilateral glutes.
But I’m attaching what he shared because it’s interesting.
