87M 2 months pain, 3 months post op Shoulder MRI. Please help

Not enough suture anchors for SCR. Maybe a subacromial spacer. They all rupture and form subacromial scar/granulation tissue after a few months. Just my thoughts. Hope that helps.

What is that blob between the joint and deep surface of the acromion?
Is it some kind of substance / structure to prevent cranial migration of the humeral head??
I’m sure someone can tell me.
