53F recovering alcoholic, Shoulder MRI, has an unusual condition and bilateral diminished range of motion


I was just called to discuss, because I had not mentioned the abundant subcutaneous fat
Apparently she has a condition called Multiple Symmetric Lipomatosis, which has an association with chronic alcohol consumption, which "may play a role in adipocyte hyperplasia in genetically susceptible individuals"
I was asked if there was a focal circumscribed lipoma; I do not see one
In person, I’m told, she’s generally very thin, with marked fat localized to her shoulders

She has bilateral capsular thickening and edema, typical of adhesive capsulitis, with clinical frozen shoulder
I only recall concomitant bilateral adhesive capsulitis in one patient with diabetes
I’ve never heard of this condition, but I cannot find an associated risk for adhesive capsulitis

She also has bilateral supraspinatus tendinosis, with partial tear on the right
