Thanks to the many people who offered differential diagnosis possibilities.
I reported that the DDx includes benign and malignant lesions, despite the chronic / indolent history.
I reported that the DDX included but was not limited to IPEH, dematofibrosarcoma protruberans, Squamous Cell Ca related to chronic scar and pilomatricoma….
additional suggestions included benign vascular malformation, hemangiopericytoma and eccrine spiroadenoma.
I emailed some clinic contact, but I am not optimistic about that….will try to make sure that the referring internist refers him for biopsy.

Just wanted to share something with you to get your input and see if there is any possibility of utilizing this particular AI in our field.
The following is from Elon Musk’s AI called Grok. I have subscribed to the Premium on his Twitter (X) which cost around $8 a month and Grok comes with it for free. I simply added Hilary’s images and asked for a differential diagnosis, and within one second I received the following.
I understand it is not perfect and it’s a WIP and it does make mistakes, but I think it’s getting better and better. I think it would be a good tool when we are pressed for time and need a quick DDX and don’t have time to do a literature search. We can simply take a picture of some key images and add it to Grok, and within a few seconds we can get a general idea of what we’re looking at.
What are your thoughts on this and cooperating with Elon to improve it?

Just to add some unsolicited balance to those remarks:
I think regardless of the utility of Grok’s differential now, it’s an attempt at progress. An attempt that can be built on. Anyone, like Elon, putting their energy toward progress is a good thing.
More good things would at least help the direction you think the world is headed!

I promised the author anonymity…but I have to share my personal all time favorite OCAD comment:
I have three opinions that I’ll share with you:
1). That’s a useless ddx.
2). The world’s going to hell in a handbasket.
3). Fuck Elon.
okay that last one is more of a sentiment than an opinion, but whatever.
My apologies to Ray for my misinterpreting the comment which may have been satirical and not political to Ray. Ray had nothing to do with it it was sent by someone else. I’ll sneak back into emeritus status and let Hilary do her phenomenal job of directing content and moderating and let these types of discussions go forward hopefully without misinterpretation.
