50M shoulder pain 1 month after heavy lifting, MRI, Please Help

There is a tear at the anterior insertion of the supraspinatus tendon

But look where the humeral head is!

I know, back in the day, people asked about the capsule possibly causing
mechanical issues in the anteroinferior joint, but I recall that we all
concluded that was a BIG nothing.
Certainly there is marked posterior subluxation of the head and the capsule
seems to just be minding its own business coursing from point A to B.

But why is the head so subluxed?
Is there a reason to recommend arthrography, or just report what I see?
Is it possible his head is always flopping around and the new symptomati
finding is the supraspinatus tear????

Don’t think I’ve run out of questions….they’ve just been piling up.
Might be another one soon….
