49M 2 months shoulder pain, to evaluate RCT. MRI. Please help

OK….overwhelming "enthusiasm" for infection. No joint space narrowing….so I’m suggesting atypical infection….such as TB, Coccy, Histo….
Many unreachable phone numbers….but I emailed the referrer and am hoping (beyond hope) that I might get follow up.

Thank you for your help

Hilary

This is not degenerative tendinosis or rotator cuff tear…
I have NO other history and no other imaging…no XRs
The degree of BME like signal is strange, as is the extend of infraspinatus muscle edema
There is alot of synovial inflammation with little or no fluid in the joint and biceps sheath
There is thickening and bright T2 signal in the supraspintues tendon and at the anterior infraspinatus MT junction

This degree of edema makes me think of crystal deposition…but I don’t see any dark signal deposits to suggest HADD (all resorbed???)….could this be gout??
Could this be some infection?
I’ve never seen tumor look like this…agree?

Thanks…I need your help.

Hilary
