42 year old with flexion problems Elbow

Hello everyone !

I bring you this case of a 42 year old male patient, who had a blocked elbow in flexion for the last 6 months. No history of trauma.

In the images i attached, you can see moderate quantity of fluid in the articulation, with synovial hypertrophy and there are 2 well circumscribed lesions, adjacent to the distal humerus epiphysis that have signals similar to cartilage. The bigger lesion has a low ring-like signal in the peripheral zone.

Also medialli next to the olecranon, also intraarticularly there are some chondral bodies with peripheral ring like low signal calcifications.

My question is that, those well circumscribed lesions are chondral bodies in a case of synovial chondromatosis ? Sometimes it’s not that easy to differentiate from PVNS.

How would you describe them ?
