35F with small lipoma in sciatic foramen.

Hello again,

Got advice on Botox injection in piriformis muscle as treatment, which also can be diagnostic.

Being aware of this finding I suddenly noticed fat deposits in the same area in other patients. Could not find any mentioning of this in articles.

Following are some pictures of other patient with similar finding of fat in greater sciatic foramen.
The first patient has the most conspicuous finding, but the others seem like the same variation.
I took a quick glance on 58 MRI pelvis in females, and found some sort of asymmetric fat in this position in 9 patients, while 49 had no fat there.

Anyone else that has encountered this finding? Any thoughts on the subject are much appreciated.
I have read on sciatic nerve variations, but I think this is something else.
Herniation of sciatic foramen is a diagnosis, but patient nr 1 has axials on the sciatic nerve that show no herniation.

Patient 2 32F

Patient 3 42F

Patient 4 51F
