My vote is for chronic mid portion ACL tear with tethering to the adjacent PCL given effacement of the intercondylar fat on coronal T1. The ACL fibers appear “continuous” but imo they are attenuated.
Is the patient Lachman positive on physical exam?

I know that the meniscal root ligament can be associated with anterior tibial translation, and can add to stress on the ACL, but can this degree of translation be attributed to the root tear alone?
Or am I missing a chronic ACL tear????
