Dr. Luis Cerezal responded:
I believe there is a tear of the volar capsular insertion of the TFCC and involvement of the deep anterior lamina of the volar distal radioulnar ligament, with distal radioulnar synovitis.
MR arthrography or CT arthrography would show it more clearly, but I think this study is diagnostic and of very good quality.

There is both dorsal and volar ulnocarpal synovitis
There is marrow edema of the distal ulnar epiphysis and synovitis in the DRUJ
There is fraying and increased intrasubstance signal with ?? partial tear of the foveal lamina of the ulnar limb TFCC
I think the GRE suggests the dorsal foveal lamina is intact…not sure about the volar insertionThe articular disc looks thinned without focal tear
Do you agree?
Would you recommend DRUJ arthrography?
What are your thoughts?Thank you.
